I have recently had problems with CB-servers 12 and 15, where I have not gotten any reply when using triggers. The triggers as !login, !goal, and !cv.
I am worried that this also have an affect on game completion and points
Server 13 seem okay, though

Yes, this is a known issue and is happening since the upgrade to the 1.11.x revision of OpenTTD. We are working on solving these issues, but basically the problem lies in the dedicated servers which we have available. All servers who have this issue regularly are hosted on a physical server which apparently can’t handle the load of all our instances which are hosted on it, resulting in losing connectivity between our controller and OpenTTD.

Anyway, we are working on it, hope to have this issue fixed soon.

In the meantime, if you encounter this issue, you can always make an !admin call (will have to join another server though, as this issue also means admin calls don't work on the affected servers). Then, if any of us are online, we can reboot the server, which usually resolves the issue.
